Ashford Musculoskeletal and Sports Physiotherapy Centre Limited which is a registered company in England and Wales Company registration: 09157278, with a registered office 25, Cranford Avenue, Staines-upon-Thames, Stanwell, TW19 7AH

1. DATA PROTECTION:
All data will be handled in accordance with the Data Protection Act, 1988. Medical records will be stored electronically and only accessed by authorised personnel. Disclosure of information may be made to appropriate health professionals, such as your GP, when communicating about your presenting condition. It is common practice to write to your GP and/or referring Specialist; if you do not want this to happen please inform your clinician.

2. CONSENT TO TREATMENT:
All assessments and treatments will be performed by a qualified, registered and insured therapist who will practice within the defined scope of practice as per the relevant governing bodies in the United Kingdom. You must advise your therapist of any factor that may affect your treatment or diagnosis. Following examination, an explanation of the therapist’s opinion and proposed treatment will be provided. It is important that you fully understand the diagnosis, recommendations, procedures and treatment techniques that may be involved, as well as any likely effects. Your therapist should make every effort to ensure you do, but if you are in doubt, please ask for further information. With certain conditions, a degree of undress may be required and this will be explained at the time by the therapist. You are entitled to be accompanied during your treatment if you wish.

4. Insurance company payments

Ashford Musculoskeletal and Sports Physiotherapy Centre Clinic is a registered provider with all the major medical insurance companies. We are prepared to invoice the insurance company direct on your behalf provided you provide us the appropriate registration/membership number and a valid claim or authorization number where required prior to your INITIAL ASSESSMENT. We are unable to invoice insurance companies retrospectively on your behalf.
Please note that you are responsible for payment where there is insufficient cover, an insurance company rejects a claim or an excess exists on your policy.
This agreement is between yourself and Ashford Musculoskeletal and Sports Physiotherapy Centre Clinic and you are ultimately responsible for all payments.

5. Payment methods

Self-paying patients are required to pay for their session in advance or on the day.
Our physiotherapists are able to receive cheques (payable to Ashford Musculoskeletal and Sports Physiotherapy Centre Ltd) and cash in clinic (we require exact change or any excess will be credited to your account) Alternatively we also accept BACS payments (please use name and session date or invoice number as reference)

6. Late payment

We request all invoices to be settled immediately on receipt. Accounts outstanding for more than payment terms or seven days following treatment will be liable to reasonable administration charges and patients will be notified via Email or letter. Failure to respond to subsequent invoice reminders will lead to your account being passed to a debt recovery service.

7. Non-attendance and cancellation

If you do not attend an appointment or cancel your appointment within the 24 hours prior to the appointment time the full charge for the appointment will be levied unless exceptional circumstance apply. This includes appointments that are re-scheduled at the discretion of the Managing Director.

8. Discharge reports

As a matter of good practice your physiotherapist may send a discharge report to your general practitioner, insurance provider or consultant. Please inform your physiotherapist if you do not wish them to do this.
